About Me card pilot



The About Me card, developed by Healthwatch Warrington, is here to support you during your hospital visits.

It lets staff know about things that might make you feel anxious, overwhelmed, or triggered during your appointment. By sharing what helps you feel safe and understood, the card helps your care team respond with kindness and compassion.

The About Me Card allows patients to share with healthcare staff things that may trigger a ‘fight, flight, or freeze’ response or make them scared or anxious when attending the hospital. This could be from things such as:

  • trauma
  • sexual assault
  • personality disorders
  • neurodiversity
  • anxiety
  • or other lived experiences that may impact the patient’s emotional regulation

Apply for an About Me card 

The About Me card is a pilot scheme for six months, running from September 2025 to February 2026. 

The card can currently only be used at Warrington Hospital for visits to the Emergency Department (ED), on wards and outpatient appointments.
