If you need to make changes to a breast screening appointment, please complete the form on the breast screening website.
If you have an outpatient appointment
Please sign up to manage your appointments online via the NHS App. This will link all your hospital and GP information in one place. You can be sent alerts and reminders with regards to your hospital appointments once you sign up and ‘switch on’ notifications.
We strongly encourage making changes to your appointments via the link within your appointment notification on the NHS App / Patient Portal, as this is the easiest and quickest way to do so. However, if you do not have access to the app, please use the form on this page.
Important information
Before cancelling an appointment it is important to know that the Trust’s Patient Access Policy states that where patients have cancelled more than two consecutive appointment offers made with reasonable notice, a consultant should undertake a clinical review and notify the patient’s GP where appropriate.
