Ward K25 - Older Persons Short Stay Unit


General introduction

Ward K25 is the Older Persons Short Stay Unit and is part of the Department of Medicine for Older People.

The ward cares for patients aged 65 years and over that present with a frailty syndrome and may be ready to be discharged to go home, or to the place they call home, within 72 hours of admission.

The purpose of Ward K25 is to enhance the patient experience and reduce the length of hospital stay using the skills and expertise of the Frailty Team.

How to access this service

Patients are transferred to Ward K25 from other wards for continuation of their care.

About the team

A multidisciplinary team of geriatricians, nurses, therapists, discharge facilitators and others will ensure patients are ready to be discharged as soon as it is safe to do so.
