Ward B3 - Halton


General introduction

Ward B3 at Halton Hospital Nightingale Building is a 'ready for discharge' ward.

The ward team provides care for patients who have been assessed as medically ready for discharge home or to another place of care. Patients may be moved to the ward while waiting for community services support to be put in place, such as a place at a care at home, nursing or residential home, or for equipment to be provided to support their recover and rehabilitation.

While you are being cared for on Ward B3, the team will continue planning for your discharge from hospital and will make any changes to discharge plans, where required.

How to access this service

Patients are referred to Ward B3 by nursing and medical staff when they are considered medically ready for discharge home from hospital or ready to be transferred to their next place of care. 

All patients referred to Ward B3 are assessed by the ward team to ensure we can meet your needs before accepting onto the ward.

About the team

The ward team consists of nurses, doctors and healthcare assistants as well as occupational therapists and physiotherapists to ensure we meet the needs of our patients and prepare for a safe discharge/transfer of care.
