Halton Community Therapy

The Halton Community Therapy team serves Halton residents aged 18 years and over, who are registered with a Halton GP and need therapy in a community setting (this could be at a patient's home or care home).
We treat patients who have:
- long term / chronic conditions
- neurological conditions
- palliative care needs
- functional impairments
- musculoskeletal conditions
- mobility problems
- orthopaedic conditions (including orthopaedic conditions following surgery)
- been discharged from hospital and need therapy in the community
- had a fall
We can help you by:
- completing a specialised home assessment and creating a patient-centred rehabilitation plan
- providing a specialist therapeutic assessment and rehabilitation
- developing personalised home exercise programmes
- improving function and promoting independence in daily living activities
- improving mobility / providing mobility aids
- facilitating discharge from hospital including working with the Neurotherapy Early Supported Discharge Service
- preventing admissions to hospital
- providing support and advice to patients and their carers
- liaising and working with other service providers and referring as appropriate
- assessing for equipment and adaptations (as part of our therapeutic intervention)
- managing pain
- therapeutic moving and handling advice for carers and family
