Halton Community Therapy


General introduction

The Halton Community Therapy team serves Halton residents aged 18 years and over, who are registered with a Halton GP and need therapy in a community setting (this could be at a patient's home or care home).

We treat patients who have:

  • long term / chronic conditions
  • neurological conditions
  • palliative care needs
  • functional impairments
  • musculoskeletal conditions
  • mobility problems
  • orthopaedic conditions (including orthopaedic conditions following surgery)
  • been discharged from hospital and need therapy in the community
  • had a fall

We can help you by:

  • completing a specialised home assessment and creating a patient-centred rehabilitation plan
  • providing a specialist therapeutic assessment and rehabilitation
  • developing personalised home exercise programmes
  • improving function and promoting independence in daily living activities
  • improving mobility / providing mobility aids
  • facilitating discharge from hospital including working with the Neurotherapy Early Supported Discharge Service
  • preventing admissions to hospital
  • providing support and advice to patients and their carers
  • liaising and working with other service providers and referring as appropriate
  • assessing for equipment and adaptations (as part of our therapeutic intervention)
  • managing pain
  • therapeutic moving and handling advice for carers and family
How to access this service

See below the referral criteria for the Halton Community Therapy Service: 

  • patients who are unable to travel to an outpatient unit for therapy due to medical reasons
  • patients who would benefit from intervention within a community setting such as their own homes, their carers’ homes, nursing homes, residential homes and / or day care facilities
  • patients must have realistic and achievable goals
  • there must be a safe home environment that is an appropriate setting for therapy / rehabilitation

Referrals are received from:

  • consultants, GP’s, community matrons, district nurses, Macmillan nurses and other therapists
  • other agencies are able to refer to the service via the patient's GP

Please note that we do not accept equipment-only referrals, referrals requesting chair assessments and / or moving and handling assessments.

The Halton Community Therapy team is an integrated multi-disciplinary team composed of physiotherapists, occupational therapists and therapy assistants.
