Cardiac Rehabilitation and Community Heart Failure


General introduction

Cardiac Rehabilitation and Community Heart Failure Nurses, support patients after a cardiac event, cardiac surgery or a new cardiovascular diagnosis (for example, heart attack, stents, bypass or valve surgery and heart failure).

The team will help in your recovery, to improve your mental and physical wellbeing or management of your condition, through:

•    education on risk factors
•    exercise prescription 
•    physio rehabilitation
•    advice and support that is tailored to your needs by our friendly and experienced team

Our community heart failure nurses can help titrate (adjust) medications and help manage your symptoms alongside your GP or cardiologist.

Our service is open Monday to Friday from 8am to 4.30pm.

Please note we are not an emergency service. In an life-threatening emergency please call 999.

How to access this service

Patients are referred to us following discharge from hospital after they have experienced a cardiac event. Patients may also be referred by their cardiologist, cardiac nurse or their GP.

We are not able to accept self-referrals.

About the team

The team consists of nurse specialists, physiotherapists, exercise physiologists plus a dietician, occupational therapist, assistant rehabilitation practitioner and are supported by administrators. 

The team is also supported by heart failure specialist nurses, cardiology specialist, consultant cardiologists and GPs with an extended role in cardiology.
