Anticoagulation Service


General introduction

The Anticoagulation Service supports adult patients receiving anticoagulation therapy within hospital and within community settings.

The care we provide includes:

  • regular blood tests for patients requiring drug level monitoring
  • discharge dosing and follow-up appointments for anticoagulated patients requiring INR monitoring by the service
  • support and advice for patients being discharged to locations outside of the Trust area
  • remote support and management of patients in partnership with district nursing teams
How to access this service

We run clinics in hospital, in community settings and over the telephone clinics from Monday to Friday and provide emergency cover over weekends and bank holidays.

The department is open Monday to Friday, 9am to 5pm.

Criteria for home visits 

Patients will be accepted for a home visit if they are housebound, and always receive home visits for GP or district nurse appointments.

Patients receiving home visits, or their carers, must be contactable by phone in case of dose changes.

Any patients found not to fit the criteria will be asked to attend clinics for future appointments. Patients that can attend GP, dental and hospital appointments will not be considered for home visits.
